chiropractic care. In order to make your visit less
time consuming, we ask that you fill out and submit
the New Patient Form, as well as download, print and
fill out the Medical History Form before you come in
for your appointment. Filling out these forms will
enable us to serve you better and faster. |

review your information. Please give us a 24 hour
notice if you need to cancel your appointment.
